Total T4 is the first-line screening and is elevated in the vast majority of hyperthyroid cats. Free T4 by dialysis is reserved for cats with compatible signs and normal or borderline total T4 because it is less specific alone. Long-acting basal insulins such as glargine combined with low-carbohydrate diets give the best glycemic control and the highest remission rates in cats. Glipizide is less effective, and regular insulin is for hospital DKA management.
