Asymmetric ataxia with focal muscle atrophy is the hallmark of EPM caused by Sarcocystis neurona. Cervical myelopathy and degenerative myeloencephalopathy cause symmetric deficits, and botulism causes diffuse flaccid weakness. Spastic paralysis with third eyelid prolapse and hyperesthesia following a wound is classic tetanus from Clostridium tetani toxin. Botulism causes the opposite picture, flaccid paralysis with weak tail and tongue tone.
