Fomepizole inhibits alcohol dehydrogenase and prevents conversion of ethylene glycol to toxic metabolites when given early (within about 8 hours in dogs). Charcoal binds ethylene glycol poorly and fluids alone do not stop metabolite formation. Factor VII has the shortest half-life of the vitamin K dependent factors, so PT prolongs first, typically 36 to 72 hours post-ingestion, before clinical bleeding. Platelets and BMBT remain normal.
