American Heartworm Society guidelines require antigen and microfilaria testing before starting prevention in dogs over 7 months old; giving preventives to microfilaremic dogs risks reactions, and undetected adult infection progresses. This is acute congestive heart failure from myxomatous mitral valve disease; emergency management is oxygen and parenteral furosemide, with pimobendan added once oral dosing is feasible. Fluids are contraindicated.
